The Licensing and Certification Unit survey sheets provided below are tools that aid LCA staff in surveys of provider facilities and/or services. They do not include all standards that are assessed at surveys. Providers are accountable for compliance with all applicable Standards and/or Regulations regarding Licensing, Certification and Criminal records Check.
